Huisartsenpraktijk de Jong en van Ham is een kleine huisartsenpraktijk met 2 huisartsen, 2 assistentes en 2 praktijkondersteuners.

We leveren laagdrempelige huisartsenzorg met aandacht voor de mens in al zijn facetten. Wij bieden basisgezondheidszorg in de volle breedte. Er is veel aandacht voor een gezonde leefstijl en eigen verantwoordelijkheid voor gezondheid. Onze praktijkondersteunster somatiek helpt bij diabeteszorg en preventie van hart en vaatziekten. Daarnaast kunt u bij onze praktijkondersteunster GGZ terecht voor psychische ondersteuning.
